That should be 12/15, not 12/16. But if you do this 4-bit to 8-bit
conversion enough times, you'll discover that you can multiply by 17
(not 16!) to go from 16 to 256 colors (and divide by 17 to go the
other way), because 255/15 = 17.
Or,
12 x 12 x
-- = --- reduces to -- = -- , x = 12 * 17 = 204.
